c 0 py <br /> Reimbursement Amount <br /> Reimbursement shall be limited to the lesser of: (1) the medical insurance <br /> premium paid by the eligible retiree, or (2) what the Port's (employer) cost is or <br /> would be to provide medical coverage for an active employee of the same age as <br /> the retiree, or (3) the insurance premium for a Medicare supplement plan at the <br /> retiree's earliest Medicare eligibility age, whether or not the retiree enrolls in <br /> Medicare. <br /> Reimbursement Process <br /> 1. All reimbursements shall be made in arrears. <br /> 2. Retiree, or his /her designee, shall submit a written request for <br /> reimbursement which shall include: <br /> (a) retiree's name and date of birth; and <br /> (b) coverage month and year; and <br /> (c) a copy of the premium statement evidencing payment of the <br /> premium. <br /> 3. Retiree shall submit reimbursement requests no less frequently than <br /> quarterly. <br /> 4. Port shall endeavor to reimburse retiree for the eligible premium amount <br /> within 21 calendar days of receipt of retiree's written request. <br /> Section 1I <br /> This Resolution does not supersede Resolution 1342 of the Port <br /> resolutions and is in addition thereto. <br /> The policy set forth in this resolution is subject to review and modification <br /> by the Board from time to time, and shall be reviewed no less frequently than five <br /> (5) year intervals by the Board. <br /> * * * * * <br /> Reso 176 2 <br /> 12/22/2010 <br /> Port <br />
