01/10/2011 <br /> furnish additional health statements from her <br /> physician upon reasonable request. <br /> 18.8.2.3 Prior to the establishment of a specific date for <br /> return to duty, the employee shall submit to the <br /> Human Resources Director a notice of intention to <br /> return to duty, accompanied by her physician's <br /> statement certifying that the employee is medically <br /> qualified to assume regular duties and <br /> responsibilities. <br /> 18.8.2.4 The Human Resources Director or his /her designee <br /> may designate the specific beginning and ending <br /> dates within the pay period requested by the <br /> employee to meet the needs of the employee and <br /> the City. <br /> 18.8.3 The employee on leave shall be returned to her original position, or <br /> if that position is not in existence, to an equivalent position within <br /> her classification. <br /> 18.8.4 A maternity leave, absent physical disability, is granted without pay <br /> for the duration of leave. The employee may elect to continue <br /> medical and dental insurance coverage for up to one (1) year <br /> during this leave at her own expense. <br /> The foregoing Maternity Leave provision shall be subject to applicable federal <br /> and state law. <br /> 18.9 Parental Leave of Absence Without Pay <br /> A male or female employee may be granted, at the Fire Chief's discretion, a <br /> leave of absence without pay to fulfill parenting responsibilities for up to one <br /> (1) year following the child's birth or one (1) year following the filing of <br /> application for adoption and actual arrival of a child in the home. <br /> The employee on leave shall be returned to his /her original position, or if that <br /> position is not in existance, to an equivalent position within his /her <br /> classification. <br /> A parental leave, absent physical disability, is granted without pay for the <br /> duration of leave.
