��p(�u,�.� ���,n n q <br /> i i•Wi"ur 4� <br /> AMENDMENT NO. 4 TO AGREEMENT FOR Profiessional Consulting Services <br /> Maximus Consul#ing Services, Inc, <br /> This Amendment Na. 4 is entered into and efFective as of the 1Sl day of July, <br /> 2014, amending the agreement dated February 14, 20'41 {the "Agreement") by and <br /> between the City of Redwood City, a charter city and mun€cipal corporation ("City"}, and <br /> Maximus Consulfiing Services, Inc., a whoily owned subsidiary of Maximus, Inc., <br /> ("Consultant") (coilectively, the "Parties")for cos#reimbursement services. <br /> RECITALS <br /> A_ On February 14, 2011, the Parties executed an Agreement for Professional <br /> Consulting Services; and <br /> B. On July 1, 2011, #he Parties executed Rmendment No. 1 to extend the term <br /> of the Agreement; and <br /> C. On Jufy 1, 2012, the Parties executed Amendment No. 2 to amend the term, <br /> scope of services and fee; and <br /> D. On July 27, 2013, the Parties executed Amendment No. 3 to amend the <br /> term, scope of services and fee; and <br /> E. The Parties have negotiated and agreed fo a supplemental scope of work <br /> and fee schedule, which is at#ached to and incarparated by fhis reference as Exhibit " <br /> A" and Exhibit" B ". <br /> N4W, THEREFORE, in consideration of these recitals and the mutual covenants <br /> contained herein, the Parties agree as follows: <br /> 1. That the Agreement, as may have been amended from time to time, is <br /> hereby extended for a period of one ysar ending on June 30, 2015 and Consultant will <br /> be paid a not to exceed amount of nine thousand five hundred dollars ($9,500). <br /> 2. Consultant will complefe all work described in Exhibi# "A" by June 30, 2015 <br /> for the campensation listed in Exhibit"B". <br /> 3. A[I other provisions of the Agreement, as may have been amended from <br /> time to#ime, wiE[ remafn in full force and effect. <br /> 4.
