�- I F-15 <br />Exhibit B <br />METHOD AND RATE OF PAYMENT TO CONTRACTOR <br />In full consideration of the services provided by the Contractor pursuant to this Agreement, the County <br />shall pay the Contractor a Quarterly Amount not to exceed $14,681.50 (see break down of costs <br />below) upon receipt of invoice. The Contractor shall submit invoices within 30 days of the end of the <br />billing period and the County shall pay the invoices within 20 working days following receipt of <br />invoice and required reports as shown below. The total amount of the contract shall not exceed <br />$58,726 except in those circumstances where match funds remain at the end of the fiscal year and not <br />to exceed $10,000 as approved by the Director of Human Services Agency or her designee. <br />Quarterly Break down of costs: <br />Funding Source Amount <br />General Funds + Community Development Block Grant (CDBG) funds: I $ 10,440.50 <br />Matching Funds not to exceed: I *$ 4,241.00 <br />Total not to exceed per quarter: $ 14,681.50 <br />* The incentive match funds will be based on the amount of new funds raised by the contractor for the FY 2003 -04 and <br />will not exceed $4,241.00 per quarter or $16,964.00 per Fiscal Year. Should there be any unused matching funds at the <br />end of the Fiscal Year, the Human Services Agency Director or her Designee may distribute the unused matching funds to <br />those Core Service Contractors who exceeded their fund raising goals.) <br />Invoices and reports to be mailed to Rosa Mendoza, HCD Specialist, 262 Harbor Boulevard, Building <br />A, Belmont, CA 94002. <br />Report required to process invoices: <br />Quarterly reports must contain the following information: <br />• Client services delivered <br />• Client demographics as required by CDBG funding (if applicable) <br />• Number of clients served in unincorporated area of Redwood City <br />• Documentation of successfully funded commitments of new or increased revenues for the <br />Fiscal Year 2003 -04 <br />13 <br />
