<br />5.3A..:¡' <br /> <br />ATTACHMENTS. The following questions should be addressed on separate pages as Attachments. Each <br />attachment must be labeled with Attachment # and Title in the upper riaht hand corner. If the Attachment <br />contains multiple pages, you should include a separate divider page labeled appropriately. If the information <br />requested is not applicable to your project, is not available, or is included as a part of another attachment, <br />insert a place holder page appropriately labeled and so indicate. <br /> <br />Attachment 1 - Program Staff. <br /> <br />Specify the names, experience, responsibilities and roles of all staff who will be responsible for the Program. <br />Indicate whether they are full-time or part-time employees. <br /> <br />, <br />Attachment 2 - Budgets. The following budget information should be limited to the specific Program for which <br />you are requesting funds and not your overall organizational budget. If overall agency administration is being <br />pro-rated to the specific program, the narrative should indicate this and include the approximate percentage of <br />proration being used. <br /> <br />Attachment 2a - Current Year (2003-04) Budget. Show all sources and uses of funding in addition to <br />San Mateo County funds. Provide a narrative explanation of each line item. This Budget is required even <br />if you did not use any San Mateo County funds. (see Funding Guidelines, Exhibit 4 for example) <br /> <br />Attachment 2b - Proposed Budget. Show all proposed sources and uses of funding in addition to San <br />Mateo County funds. Provide a narrative explanation of each line item. <br /> <br />Attachment 2c - Other Funds. For all other funding sources, attach documentation of funding <br />commitments. Indicate the status of funding requests if you do not have firm commitments. <br /> <br />Attachment 3 - Project Timeline. Provide a program schedule for the use of requested funds including <br />specific work tasks, projected delays, accomplishments and major milestones. <br /> <br />Attachment 4 - Community Notification. If you are proposing a new program to be operated in one of the <br />cities, you must notify that respective city through the office of the City Manager. Please provide evidence of <br />this contact and any acknowledgement by the city. <br /> <br />3 <br />
