TENTATIVE AGREEMENT BETWEEN THE CITY OF REDWOOD C 6.1.13. -Page 19 <br />AND REDWOOD CITY POLICE SERGEANTS ASSOCIATION (PSA) <br />Effective January 1. 2012. City contributions to the Cafeteria Plan will be limited <br />to contributions described in Section 10.2.2 and will not include credit to a <br />flexible spending account. <br />10.33 Dental <br />The City shall continue to provide to eligible employees and dependents fully paid dental <br />insurance through Delta Dental Fee for Service program <br />agFeement, or upon the establishment of a mutually agreed upon alternative program, to <br />include coverage's as follows: $21-,000 annual cap for basic coverage and $2,5000 <br />lifetime cap for orthodonture. <br />Effective January 1. 2012, the City will pay ninety percent (90 %) of the dental insurance <br />premium for eligible employees and dependents. <br />10.43 Life Insurance <br />Effective September 1. 2011, t The City shall 89AtiRUe t9 GffeFprovide to eligible <br />employees the —life insurance coverage of $50.000. , <br />1974. <br />10.70 Vision <br />Effective with the first full pay period on or after September 1, 2003, the City will contract <br />with Vision Service Plan (VSP) or comparable vision care provider to provide vision care <br />benefits for employees and their dependents. The Vision Service Plan 'B' provides for <br />an exam every 12 months, lenses every 12 months if needed, and frames every 2 years, <br />if needed. There is no deductible for employees, but a $20.00 per person deductible will <br />apply to dependents each time benefits are available and will be paid by employees. <br />Effective January 1. 2012. the City will pay ninety percent (90 %) of the vision insurance <br />premium for eligible employees and dependents. <br />June 3, 2011 —City <br />Page 8 <br />
