<br /> 6.1G <br /> Page 17 <br /> Exhibit F <br /> HUD Required Quarterly Reports <br /> City of Redwood City - Fair Oaks Community Center <br /> Core Services FY 2006-07 <br /> No request for reimbursement will be honored until all reports required hereunder have been <br /> received by the Department of Housing. <br /> Contractor shall submit to the Human Services Agency the following reports within 30 days <br /> of the end of each quarter: <br /> 1. Performance Log (optional), to be submitted on form provided by the County Human <br /> Services Agency (HSA), which shaH provide income and demographic information of <br /> each individual or household served as well as a brief description of the services <br /> provided during the quarter. <br /> 2. Performance Summary, to be submitted on form provided by the County HSA, which <br /> shall summarize the number of clients served by their ethnicity and income. <br /> 3. Consolidated Funding Source Report. <br /> 4. Quarterly Proiect Narrative Report. which provides a narrative of highlights/details of <br /> both the funded activity and of the Contractor's organization. <br /> Performance reporting forms will be provided to Contractor by the County of San Mateo <br /> Human Services Agency Electronically. <br />
