The following must be included: <br /> - State nature of problem and what occurred <br /> - When leak/problem occurred <br /> - Proof of repair (copy from plumber, repair person or receipts) <br /> . The information will be reviewed to see if a credit request is valid. Customers <br /> will be notified either in writing or by phone if their credit request is denied. <br /> . Customers should send in partial payment with written request. The partial <br /> payment may be based on average past utility bills. <br /> - <br />
