<br /> 6.1F <br /> Page 3 <br /> 8/28/06 <br />FUNDING A G R E E M E N T FOR HUMAN SERVICES FINANCIAL ASSISTANCE PROGRAM <br /> <<AGENCY>> <br />THIS AGREEMENT, made and entered into this day of , 2006, by and <br />between the CITY OF REDWOOD CITY, a municipal corporation of the State of California ("City") and <br /><<agency>>, a California nonprofit organization ("Contractor"); <br /> WIT N E SSE T H: <br />WHEREAS, Contractor provides <<provides>>; and <br />WHEREAS, Contractor has requested financial assistance in order to conduct the program for <br />residents of City during the fiscal year commencing July 1, 2006, and ending June 3D, 2007; and <br />WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum of <br /><<amount>> ; <br />NOW, THEREFORE, the parties hereto do hereby agree as follows: <br />1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of <<amount>> for <br />exclusive use by Contractor during the fiscal year commencing July 1, 2006, and ending June 30, 2007, <br />solely for the purposes described in paragraph 2, <br />2. USE OF FUNDS. Contractor shall use the funds provided pursuant to paragraph 1 solely <br />for the Program purposes described in Exhibit "An (Contract Objectives) attached hereto and by this <br />reference incorporated herein. <br />3. CHANGES TO PROGRAM. No changes in the Program described in this agreement <br />which are funded by the financial assistance provided under paragraph 1 shall be made without the prior <br />written consent of City. <br />4. PAYMENTS. Contractor shall keep detailed and accurate records of all expenditures made <br /> and expenses incurred which are funded under this agreement. Payments of funds <br /> - <br />
