All terms, conditions and services not described within this proposal but otherwise required during the course of this project will be in accordance with our 1996-97 Fee Schedule. The proposed unit rates will be in effect through February 28, 1998. Thereafter, the unit rates are subject to an increase of 5 1/2 percent per year to mitigate the annual operating cost increases. Show-up Time (min. 2 hr. increments) Bill 2 hours Work 0 - 4 hours Bill 4 hours Work 4 - 8 hours Bill 8 hours Swing/Night Shift $5/hr Additional Over 8 hours & Saturdays 1.5 x Basic Rate Sundays/Holidays & Over 8 hours on Saturday 2 x Basic Rate Disuatch/Miscellaneous We request 24 hours advance notice (minimum) when dispatching. Contractor to provide safe access to all actual inspection/testing locations (via suitable ladders, etc.). Verification of building's plumbness/levelness by others. All charges are portal to portal. Final Affidavit The final inspection report (affidavit) will be issued on the 6th working day following the request for the affidavit by the project representative. At the time of this transaction, all outstanding issues of inspection, testing and accounting (fees) shall have been cleared to our satisfaction prior to release of the affidavit. Outstanding issues include, but are not limited to, the approved resolutions to NCR' s, RFI' s, tardy submittals, 28-day concrete test results and/or payment for services. Payment Schedule Net 30 days. A 1-1/2% per month late payment charge will be levied on invoice payments received after 30 days of invoice date.
