Red — wood` Founde rauntletl 1881 Council Benefits Summary Public Employees Retirement System (PERS) — Council Members shall be entitled to participate in the PERS Retirement System. The City will contribute both the employee and the employer's portion. Health Insurance — Council Members shall be entitled to health insurance provided by the City. The plans offered shall be those of the California Public Employees Retirement System. Flexible Benefit Plan - City Council Members shall be entitled to participate in the Flexible Benefit program. The City shall contribute amount equal to the Kaiser Family Rate to establish the Flexible Benefit Plan for the purpose of payment for PERS Medical Premiums, Dependent Care, Health Care Reimbursement and the Management Cash Option (as described below). ♦ If the City Council Member elects no health plan coverage as a health savings/cash option the City shall share equally the amount designated as the City's actual maximum monthly contribution. ♦ If the City Council Member elects coverage in health, medical spending and /or dependent care the City and the Council Member will share equally in the difference between the elected coverage and the monthly contribution to a maximum of $102.50 Dental Insurance — City Council Members shall be entitled to participate in dental insurance provided by the City with an annual cap of $2,000 for basic coverage and $2,500 lifetime cap for orthodontics coverage. Life Insurance — City Council Members shall be entitled to Life Insurance with limited principal amount of $3,000.00 Vision Care Insurance — City Council Members shall be entitled to the Vision Care Service Plan for self and dependents. Long Term Disability — City Council Members shall be entitled to long term disability coverage. The city will pay the full cost of the basic rate. Mileage Reimbursement — Council Members shall be reimbursed at the prevailing IRS rate for travel requested and authorized by the City. 1 4 40 3
