(Provide 109 individuals 6/30/12 <br /> ongoing support, through weekly case meetings and workshops, for <br /> clients to improve their finance and budgeting skills and to access job <br /> search and development resources, so that participants' median income <br /> will increase by at least 20% behveen program entrance and exit.) <br /> HOM 5: Prevent homelessness. (Provide individuals in the short-term 76 individuals 6/30/12 <br /> sheiter program with assessments, targeted case management, <br /> information, referrals, and on-site services on an ongoing basis, to enable <br /> at least 70% of clients completing the program to move on to more stable <br /> or permanent housing.) <br /> HOM 5: Prevent homelessness.
