'' EXHIBIT A- SCOPE OF SERVICES, TIME AND FEE SCHEDULES <br /> Scoqe of Work <br /> Smith-Emery Company (Consultant) shall provide all required labor, material, and equipment to conduct <br /> geotechnical inspection and testing services for the Sanitary Sewer and Water Replacement Project <br /> (Redwood Avenue, Norman Court and Ebener St.). <br /> The Consultant shall: <br /> 1. Perform laboratory material acceptance standard test which includes a gredation test, <br /> compaction curve, R-Value test, sand equivalent test, and durability index test. <br /> 2. Perform Feld observations/Feld density compaction testing services (nuclear gauge) as <br /> requested, and on a intermittent basis for trench bedding and backfill in compliance with the <br /> project plans and specifications. <br /> 3. Document test data and observations through daily and end-of- job summary reports. Daily <br /> reports shall be submitted to the City on a daily basis. Daily reports should describe the <br /> activities of the contractor at the time of your field services, and the locations and results of <br /> your nuclear field density tests. <br /> Deliverables: Summary Inspection Reports <br /> Time Schedule <br /> The schedule for the work shall be one hundred fifty (150) calendar days from notice to proceed. <br /> Fee Schedule <br /> Consultant shall be paid an amount not to exceed $9,841.00 for the project as follows: <br /> SESF Personnel and Field Testing <br /> EngineeringTechnician-RegularTime $ 81.00/hr <br /> SESF Laboratory Testing <br /> Gradation Test 5 76.00 each <br /> Compaction Curve $ 118.00 each <br /> R-Value Test $ 128.00 each <br /> SandEquivalentTest $ 60.00each <br /> Durability Index Test $ 63.00 each <br />
