ORIGINAL <br /> FUNDING AGREEMENT FOR HUMAN SERVICES FINANCIAL ASSISTANCE PROGRAM <br /> BETWEEN THE CITY OF REDWOOD CITY <br /> AND <br /> PENINSULA VOLUNTEERS. INC <br /> THIS AGREEMENT, made and entered into this day of ��2011, <br /> by and between the CITY OF REDWOOD CITY, a charter c�ty and a municipal corporation of the State of <br /> California ("Cdy") and PENINSULA VOLUNTEERS, INC , a California nonprofit organization <br /> ("Contractor'), <br /> WITNESSETH <br /> WHEREAS, Contractor provides hot meals to homebound members of the community who are <br /> unable to cook for themselves, and ' <br /> WHEREAS, Contractor has requested financial assistance in order to conduct the program for <br /> residents of City during the fiscal year commencing July 1, 2011, and ending June 30, 2012, and <br /> WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum of <br /> Twenty Five Thousand and No/100 Dollars ($25,000 00), <br /> NOW, THEREFORE, the parties hereto do hereby agree as follows <br /> 1 FINANCIAL ASSISTANCE City shall allocate to Contractor the sum of Twenty Five <br /> Thousand and No/100 Dollars ($25,000 00) for exclusive use by Contractor during the fiscal year <br /> commenang July 1, 2011, and ending June 30, 2012, solely for the purposes described in paragraph 2 <br /> 2 USE OF FUNDS Contractor shall use the funds provided pursuant to paragraph 1 solely <br /> for the Program purposes descnbed m Exhibd "A" (Contract Ob�ectives) attached hereto and by this <br /> reference mcorporated herein <br /> 3 CHANGES TO PROGRAM No changes in the Program described in this agreement <br /> which are funded by the finanaal assistance provided under paragraph 1 shall be made without the prior <br /> wntten consent of Ciry <br /> 4 PAYMENTS Contractor shall keep detailed and accurate records of all expenditures <br /> made and expenses incurred which are funded under this agreement Payments of funds allocated <br />
