. � ORIGINAL , FUNDING AGREEMENT FOR HUMAN SERVICES FINANCIAL ASSISTANCE PROGRAM BETWEEN THE CITY OF REDWOOD CITY ; AND STARVISTA THIS AGREEMENT, made and entered into this day of ��`�11, by and between the CITY OF REDWOOD CITY, a charter city and a municipal corporation of the State of California ("City") and STARVISTA, a California nonprofit organization ("Contractor"), WITNESSETH WHEREAS, Contractor provides multiple hotlines including a 24-hour swade and cnsis hotline a 24-hour drug and alcohol hotline, and a parent support hotline, and on-call counselors who are able to respond to crisis situations in schools, and WHEREAS, Contractor has requested finanaal assistance in order to conduct the program for residents of City dunng the fiscal year commencing July 1, 2011, and ending June 30, 2012, and WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum of Ten Thousand and No/100 Dollars ($10,000 00), NOW, THEREFORE, the parties hereto do hereby agree as follows 1 FINANCIAL ASSISTANCE City shall ailocate to Contractor the sum of Ten Thousand and No/100 Doilars ($10,000 00) for exclusive use by Contractor during the fiscal year commencing July 1, 2011 and ending June 30 2012 solely for the purposes described in paragraph 2 2 USE OF FUNDS Contractor shall use the funds provided pursuant to paragraph 1 solely for the Program purposes described in Exhibit "A" (Contract Ob�ectives) attached hereto and by this referenceincorporated herem 3 CHANGES TO PROGRAM No changes �n the Program described in this agreement which are funded by the financial assistance provided under paregraph 1 shall be made without the prior wntten consent of City 4 PAYMENTS Contractor shall keep detailed and accurate records of all expenditures made and expenses incurred which are funded under this agreement Payments of funds allocated
