California Department of Public Health - Special Terms and Conditions . Exhibit D(F) <br /> (a) A cumulative increase of $25,000 or more in the amount paid or expected to be paid for <br /> influencing or attempting to influence a covered federal action; <br /> (b) A change in the person(s) or individuals(s) influencing or attempting to influence a covered federal <br /> action; or <br /> (c) A change in the officer(s), employee(s), or member(s) contacted for the purpose of influencing or <br /> attempting to influence a covered federal action. <br /> (4) Each person (or recipient) who requests or receives from a person referred to in Paragraph a(1) of <br /> this provision a contract or agreement, subcontract, grant or subgrant exceeding $100,000 at any tier <br /> under a contract or agreement, or grant shall fle a certification, and a disclosure form, if required, to <br /> the next tier above. <br /> (5) All disclosure forms (but not certifications) shall be forwarded from tier to tier until received by the <br /> person referred to in Paragraph a(1) of this provision. That person shall forward all disclosure forms <br /> to CDPH Program Contrect Managec <br /> b. Prohibition <br /> Section 1352 of Title 31, U,S.C., provides in part that no appropriated funds may be expended by the <br /> recipient of a federal contract or agreement, grant, loan, or cooperative agreement to pay any person for <br /> influencing or attempting to influence an officer or employee of any agency, a Member of Congress, an <br /> officer or employee of Congress, or an employee of a Member of Congress in connection with any of the <br /> follpwing covered federal actions: the awarding of any federal contract or agreement, the making of any <br /> federal grant, the making of any federal loan, entering into of any cooperative agreement, and the <br /> extension, continuaTion, renewal, amer.dment, or modiflcation of any federal contract or agreement, grant, <br /> loan, or cooperative agreement <br /> CDPH Exhibit D(F) (8/10) Page 22 of 25 <br />
