� BAAQMD FYE 2012 TFCA Funding Agreement <br /> ATTACHMENT C <br /> MONITORING OF PROJECT PERFORMANCE <br /> I. Semi-annual Reports (Section II.8): The Project Sponsor shall submit Semi-annual RepoRs to <br /> the Air District sumniarizing Project progress. Semi-annual Reports shall be prepared on the Air <br /> DistricYs Semi-annual Report form. <br /> Due Dates: Beginning 60 days after the Effective Date, every April 15, and October 15, <br /> following the Effective Date until the Final Report has been submitted. <br /> 2. Final Report (Section IL8): <br /> Due Date: March 31, 2013 <br /> The Final Report shall include the following information: <br /> A. A description of the shuttle service provided including: <br /> i. The results of a District-approved ridership survey for the shuttle routes listed in <br /> Attachment B. The ridership survey must indude the questions provided in the Air District <br /> Survey Form. These questions will include, but are not limited to, prior mode of choice for <br /> riders using the shuttle and the commute distance. <br /> ii. The following data far the Project: <br /> (1) route map, including service stops and schedule information; <br /> (2) total monthly boardings; <br /> (3) average daily boardings; <br /> (4) total number of days of service; <br /> (5) revenue miles of service; and <br /> (6) model year, engine type, fuel type, gross vehicle weight, retrofit device used for any <br /> diesel vehicles, and passenger capacity of the vehicles used to provide the shuttle <br /> service. <br /> B. A discussion of any pertinent issues or problems that arose during Project operation, plans to <br /> alter and/or continue the shuttle service, and the proposed funding sources for continued <br /> service. <br /> C. Documentation that the Project Sponsor has acknowiedged the Air District as a Project funding <br /> source during the Project Useful Life, including photographs of vehicles operated as part of the <br /> Project with Air District logos attached; documentation of use of the logo on the Yroject <br /> Sponsor's website, promotional materials, and on Project transit schedules, brochures, <br /> handbooks, and maps that promote or inform the public about the Project services; and copies <br /> of press releases and newsletter articles related to the Project (Section II.11). <br /> TFCA Regional Fund Project 11 R07 Page 13 <br />
