FXS:dmp 1~/06/94 (003/40) <br /> <br /> 0 R I G I N A L 12/19/94 <br /> RESOLUTION NO. 12379 <br /> <br /> RESOLUTION APPROVING AND AUTHORIZING EXECUTION OF <br /> AMENDMENT TO AGREEMENT FOR EMPLOYEE ASSISTANCE PROGRAM <br /> SERVICES <br /> <br /> (OCCUPATIONAL HEALTH SERVICES, INC.) <br /> <br /> WHEREAS, pursuant to the provisions of Resolution No. 12215, <br />adopted June 27, 1994, this Council approved an Agreement for <br />Employee Assistance Program Services; and <br /> <br /> WHEREAS, an Amendment to the Agreement for Employee <br />Assistance Program Services has been presented to, and reviewed <br />by, this Council; <br /> <br /> NOW, THEREFORE: <br /> <br /> BE IT RESOLVED BY THE COUNCIL OF THE CITY OF REDWOOD CITY, <br />AS FOLLOWS: <br /> <br /> That certain Amendment to Agreement dated as of December , <br />1994, by and between the City of Redwood City, a municipal <br />corporation of the State of California, and Occupational Health <br />Services, Inc., a California corporation entitled "Amendment to <br />Agreement For Employee Assistance Program Services", providing <br />personal counseling services for employees of the City of Redwood <br />City, a copy of which Amendment is on file in the office of the <br />City Clerk, to which copy reference is hereby made for the full <br />particulars thereof, is hereby approved, and the City Manager and <br />the City Clerk are hereby authorized to execute, and attest <br />thereto, respectively, said Amendment for and on behalf of the <br />City of Redwood City. <br /> <br /> <br />
