This <br /> document must be reviewed and adopted by your boarci of clirectors. 'Please sign and return the <br /> original no later than 6/1/2012 to: <br /> United Way of the Bay Area <br /> Attention: Grants Information Officer <br /> 221 Main Street, Suite 300 <br /> San Francisco, CA 94105 <br /> Funding Area: Community School Initiative <br /> Project Title: Parent Engagement and Leadership - Redwriod City 2020 <br /> Grant Amount: $ 20,000 <br /> Payment Schedule: 2 Allocations <br /> Grant Period:6/1/2012 to 5/31/2013 <br /> Grantee acknowledges and agrees that UWBA, at its �ole ciis�r�tion, shall: <br /> • Conduct a fundraising campaign to support the outcomes identified for community impact in <br /> Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, and Solano counties; <br /> • Include Grantee in specific promotions and printed materials; <br /> • Communicate the outcomes achieved by Grantee to U\JUBA, UWBA's donors and the community. <br /> • Payments on the grant will be made in 2 installments during the grant period as follows: 60% <br /> payment in month 1, or upon receipt of all required documents; 40% payment in month 7 (upon <br /> site visit completion and program officer approval). Payrrients are issued between the 15th and <br /> 30th of the month, based on the availability of funds and whether ths requirements listed below <br /> are met by the dates requested. <br /> The Grantee, in accepting funds from UWBA, agrees to adhere to all the terms and conditions <br /> contained in this MOU, including: <br /> DELIVERY OF PROGRAM ACTIVITIES AND OUTCOMES: <br /> Completion of all program activities and delivery of all program outcomes as summarized in Section 2: <br /> Activities and Outcomes Report (page 4) of the final repQrt for�m (att�ched). Modification in projected <br /> activities or outcomes or change in the use of UWBA fun�i:; is pi�ohibited without prior written approval by <br /> UWBA.
