HOM 1: Increase the number of transitional and permanent supportive 15,households 6/30/12 housing units for the homeless. (Provide safe shelter, basic necessities, and targeted supportive services for homeless families.) Other secondary goals, collaborative actions, outreach, and emphasis to be conducted. HOM 4: Meet the human services needs of the homeless. (Provide 10 households 6/30/12 ongoing support, through weekly case meetings and workshops, for clients to improve their finance and budgeting skills and to access job search and development resources, so that participants' median income will increase by at least 20% between program entrance and exit.) HOM 5: Prevent homelessness. (Provide families with assessments, 10 households 6/30/12 targeted case management, a comprehensive children's program, information, referrals, and on-site services on an ongoing basis, to enable at least 80% of clients completing the program to move on to permanent housing.)
