(Provide safe shelter, basic necessities, <br /> and targeted supportive services for homeless families.) <br /> Other secondary goals, collaborative actions, outreach, and emphasis to be conducted. <br /> HOM 4: Meet the human services needs of the homeless. (Provide 10 households 6/30/12 <br /> ongoing support, through weekly case meetings and workshops, for <br /> clients to improve their finance and budgeting skills and to access job <br /> search and development resources, so that participants' median income <br /> will increase by at least 20% between program entrance and exit.) <br /> HOM 5: Prevent homelessness. (Provide families with assessments, 10 households 6/30/12 <br /> targeted case management, a comprehensive children's program, <br /> information, referrals, and on-site services on an ongoing basis, to <br /> enable at least 80% of clients completing the program to move on to <br /> permanent housing.) <br /> ' Primary quantitative contract goal is the number of households/persons you will assist under this <br /> contract as stated in your grant application.
