� �:�IGll��i� AMENDMENT NO. 4 TO EXTEND AND AMEND THE AGREEMENT FOR PROFESSIONAL SERVICES (Shums Coda 8�Associates, Inc.) This Amendment No. 4 is entered into and effective as of the � day of , 2014, extending and amending the Agreement for Professional Servi d ed May 21, 2012 (the "Original Agreement") by and between the City of Red d City, a charter city and municipal corporation of the State of California ("City"), and ums Coda &Associates, Inc, ("Consultant") (collectively, the "Parties"). REC_ A. On May 21, 2012, the Parties executed the Original Agreement. On February 13, 2013, the Parties executed Amendment No. 1 to Extend and Amend the Original Agreement. On July 8, 2013, the Parties executed Amendment No. 2 to Extend and Amend the Original Agreement. On January 30, 2014, the Parties executed Amendment No. 3 to Extend and Amend the Original Agreement. The Original Agreement, Amendment No. 1, Amendment No. 2 and Amendment No. 3 are collectively referred to as the "AgreemenY'; and B. The Parties have negotiated and agreed on amendments attached to and incorporated in by reference as Exhibit "A", Scope of Services and Fees and to extend the Agreement for a period of six months. NOW, THEREFORE, in consideration of these recitals and the mutual covenants contained herein, the Parties agree as follows: 1. The Agreement, is hereby extended for a period of six months ending on December 31, 2014. 2. In addition to those services contained in the Agreement, Consultant will provide those services described in Exhibit "A". 3. All other provisions of the Agreement, shall remain in full force and effect. 4. All requisite insurance policies to be maintained by the Consultant pursuant to the Agreement, shall include coverage for this Amendment.
