EXHIBIT A <br /> <br /> Center for Independence of the Disabled <br /> Housing Accessibility Modification <br /> <br /> FISCAL YEAR 2003/2004 <br /> <br /> Contractor shall use the funds provided hereunder for the advancement of programs <br /> which provide housing and human services assistance to very Iow and lower income <br /> residents of the City. Contractor shall coordinate its activities with similar programs <br /> conducted by the City or with other organizations which' provide housing and <br /> supportive services to Redwood City residents, and which are consistent with the <br /> Consolidated Plan Objectives to be met during 2003/2004. Contractor shall <br /> collaborate with similar programs in order to maximize these programs and services, <br /> and the City's financial resources which are available to lower and very Iow income <br /> residents. <br /> <br /> Funding provided under this contract covers billing periods from July 2003 to April <br /> 2004. Requests for reimbursement must be submitted on a monthly basis. The final <br /> billing for April 2004 must be submitted no later than May 20, 2004.
