<br /> Exhibit "A" <br /> SUPPLEMENTAL SCOPE OF SERVICES AND FEES <br />TASK 1: THE FORUM AT REDWOOD CITY (SERIES OF 8 ADDITIONAL EVENTS) <br />Working with staff, Consultant will develop, plan and staff a series of eight (8) additional <br />educational forums, building on the sessions held between December 2003 and May 2004. <br />Consultant's staff will be available on site at the forums to facilitate and help ensure that the <br />events run smoothly. <br />Consultant responsibilities: <br />· Work with Agency to identify and select the topics and potential speakers for the forums; <br />· Invite and confirm the guest speakers, including coordinating with them about their <br /> presentation, assisting with any travel arrangements, and collecting their bios; <br />· Prepare and update a simple, one-page flier to be used as an invitation to each forum; <br />· Work the Agency to coordinate meeting logistics, including additions to the mailing list; <br />· Prepare meeting materials including an agenda, sign-in sheet and evaluation form; <br />· Facilitate (if needed) and staff the forums; <br />· Prepare a brief written summary of the forum results; and <br />· Draft speaker thank you letters for the Agency to sign. <br />Agency responsibilities: <br />· With guidance from Consultant, coordinate all logistics for the forums, including securing <br /> meeting location, coordinating room set-up, making catering and/or dinner <br /> arrangements, and securing any needed audio-visual equipment (it is assumed that the <br /> Agency will pay for any costs incurred for any of these items); <br />· Produce and maintain a mailing list; <br />· Reproduce and mail or distribute the invitation fliers; <br />· Collect and process any registrations/RSVPs for the forum if needed; <br />· Duplicate any handout materials; and <br />· Assist with check-in and on-site logistics the day of the event. <br />COST ESTIMATE FOR TASK 1 <br />Professional Time $24,000.00 <br />Direct Costs 1,120.00 <br />TOTAL TASK 1 $25.120.00 <br />The budget listed above reflects a not-to-exceed fee. The allocation of funds within that amount <br />will be adjusted in accordance with work actually performed. Consultant will not exceed the <br />amount contained in Amendment No.2 unless Agency authorizes a change to the scope. <br />. <br />
