Exhibit "A" SUPPLEMENTAL SCOPE OF SERVICES AND FEES TASK 1: THE FORUM AT REDWOOD CITY (SERIES OF 3 ADDITIONAL EVENTS) Working with staff, Consultant will develop, plan and staff a series of three (3) additional educational forums, building on the sessions held in December 2003, January 2004 and February 2004. Consultant's staff will be available on site at the forums to facilitate and help ensure that the events run smoothly. Consultant responsibilities: . Work with Agency to identify and select the topics and potential speakers for the forums; . Invite and confirm the guest speakers, including coordinating with them about their presentation, assisting with any travel arrangements, and collecting their bios; . Prepare and update a simple, one-page flier to be used as an invitation to each forum; . Work the Agency to coordinate meeting logistics, including additions to the mailing list; . Prepare meeting materials including an agenda, sign-in sheet and evaluation form; . Facilitate (if needed) and staff the forums; . Prepare a brief written summary of the forum results; and . Draft speaker thank you letters for the Agency to sign. Agency responsibilities: . With guidance from Consultant, coordinate all logistics for the forums, including securing meeting location, coordinating room set-up, making catering and/or dinner arrangements, and securing any needed audio-visual equipment (it is assumed that the Agency will pay for any costs incurred for any of these items); . Produce and maintain a mailing list; . Reproduce and mail or distribute the invitation fliers; . Collect and process any registrations/RSVPs for the forum if needed; . Duplicate any handout materials; and . Assist with check-in and on-site logistics the day of the event. COST ESTIMATE FOR TASK 1 Professional Time $7,605.00 Direct Costs 402.00 TOTAL TASK 1 $8.007.00 The budget listed above reflects a not-to-exceed fee. The allocation of funds within that amount will be adjusted in accordance with work actually performed. Consultant will not exceed the amount contained in Amendment No.1 unless Agency authorizes a change to the scope.
