EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Youth Leadership Institute (YLI) will support Redwood City 2020 as the lead Healthy <br /> Corner Store Partner. <br /> In addition YLI will: <br /> • Act as the adult adviser to YLI in leading Healthy Corner Store Makeover (HCSM) <br /> Campaign efforts in Redwood City/North Fair Oaks <br /> • Complete 2 HCSM by June 2012 and 4 by June 2013 and document steps taken to <br /> achieve the changes <br /> • Provide technical assistance and support to Corner Stores adopting Healthy Corner <br /> Store Model. <br /> • Work with Redwood City 2020 and San Mateo County Health System to identify long- <br /> term sustainable change and evaluation measures <br /> • Work with corner storeowners and partners to create incentives for owners to become <br /> Campaign partners. <br /> • Take the lead in creating promotional and policy development language for <br /> sustainability of Corner Store Model. <br /> • Complete all reporting requirements <br /> REPORTING SCHEDULE: <br /> Progress Report due to Funder by 12/1/12 (due to RC2020 for review by 11/1/2012) <br /> Final Report due to Funder by 12/1/13 (due to RC2020 for review by 11/1/2013) <br /> ATTY/AGR/2012.088/AGREEMENT BTWN RWC&YOUTH LEADERSHIP INSITUTE <br /> REV:07-02-12 PT Page 10 of 11 <br />
