,• <br /> EXHIBIT "A" <br /> PROJECT DESCRIPTION: <br /> Construction support and construction inspection services for Redwood Creek Channel Wall <br /> Replacement Project <br /> SCOPE OF WORK: <br /> Consultant shall provide the following services. <br /> TASK 1. Structural Construction Support Services, including: <br /> a. Assist the City with responses to structural related City and Contractor <br /> Requests for Information (RFIs) <br /> b. Assist the City with review of structural Shop Drawing Submittals including: <br /> i. Contractor shoring <br /> ii. Bar reinforcing steel shop drawings <br /> iii. Cast-in-place concrete mix designs <br /> iv. Shotcrete mix designs <br /> v. Certificates of compliance <br /> c. Prepare record drawings at the conclusion of construction based on one <br /> master redline mark-up provided by the City's Resident Engineer. <br /> TASK 2. Structural Construction Inspection Services for Shotcrete, including: <br /> a. Provide full time inspection during the placing of all shotcrete <br /> b. Sample fresh shotcrete <br /> c. Test slump <br /> d. Test shotcrete temperature <br /> e. Field cure test panels. Four 3-inch diameter cores or 3-inch cubes shall be <br /> cut from each panel for testing <br /> f. Check completed shotcrete work visually for reinforcing bar embedment, <br /> voids, rock pockets, sand streaks and similar deficiencies by examining a <br /> minimum of three 3-inch cores (by the Contractor) <br /> g. Test shotcrete samples <br /> DELIVERABLES: <br /> TASK 1. Consultant shall provide one (1) hardcopy of record drawings to City within 14 days, <br /> or later as deemed practical by the City's Resident Engineer, after conclusion of construction. <br /> TASK 2. Consultant shall provide inspection reports within 14 days, or later as deemed <br /> practical by the City's Resident Engineer, after the date of inspection. <br /> COMPENSATION: <br /> Consultant shall perform the Scope of Work, including expenses, for a set fee described as <br /> follows and payable within 30 days after city's receipt of acceptable deliverables. <br /> TASK 1. $10,500.00 <br /> TASK 2. 6,500.00 <br /> TOTAL $17,000.00 <br /> ATTY/AGR/2012.089/BRIGGS CARDOSA ASSOCIATES INC. <br /> REV:07-02-12 VR Page 10 of 10 <br />
