Page 1 of 4 <br /> AGREEMENT FOR SERVICE BETWEEN THE REDWOOD CITY <br /> SCHOOL DISTRICT AND CITY OF REDWOOD CITY <br /> This ageement is made as of this day of June 27, 2012 by and between the Redwood City <br /> School District("RCSD") and City of Redwood City("CONTRACTOR")to provide services. <br /> NOW, THEREFORE, THE PARTIES AGREE AS FOLLOWS: <br /> 1. SERVICES <br /> The CONTRACTOR agrees to provide said services to the RCSD in accordance with the <br /> terms and conditions of this agreement. <br /> A. Scope of Services <br /> The CONTRACTOR agrees to provide extended day academic and enrichment <br /> services described in RCSD Extended Day Program & School Minimum <br /> Expectations in Attachment A and RCSD Extended Day Policies and Protocols in <br /> Attachment B at Fair Oaks, Garfield, Hawes, Henry Ford, Hoover, John Gill, and <br /> Selby Lane. The RCSD will provide the necessary supports to the CONTRACTOR <br /> as described in Attachment A. The CONTRACTOR, if applicable will refer 21 St <br /> Century Community Learning Center families to existing literacy and related <br /> educational development services. <br /> The CONTRACTOR agrees to utilize the computer system (CitySpan) provided by <br /> the RCSD to enter daily attendance. Any technical difficulties associated with <br /> utilizing this system will be reported in a timely manner within 72 hours. <br /> 2. EQUII'MENT AND MATERIALS <br /> CONTRACTOR shall provide all equipment, materials, and supplies necessary for the <br /> performance on the agreement. <br /> 3. USE OF SUBCONTRACTORS <br /> CONTRACTOR shall not assign this agreement or any portion thereof to a third party <br /> without the prior written consent of RCSD. <br /> 4. LICENSES AND PERMITS <br /> CONTRACTOR shall obtain and keep in force all licenses, permits, and certificates <br /> necessary for the performance of this agreement. <br /> 5. INSURANCE <br /> CONTRACTOR understands the RCSD does not provide liability, malpractice, or <br /> workers compensation insurance for the CONTRACTOR. The CONTRACTOR is <br /> responsible for the maintenance of personal and professional insurance to provide <br /> protection for any claims arising out of service under this agreement. The <br /> CONTRACTOR assumes all liability for services provided under this contract. The <br /> CONTRACTOR will provide proof of insurance and will identify RCSD as an additional <br /> insured. <br /> Initial�i��1 /__��-!'y <br />
