City of Redwood City Allocation of Funding for PE+ Program SHD Fiscal Year 2014/2015 <br /> Grant Number: 2014-15- 100 <br /> Grant Amount: $317,994 <br /> Grant Period: July 1, 2014—June 30,2015 <br /> Payment Schedule: <br /> First payment upon approval of this Agreement by CITY's City Council--$136,690 <br /> Second payment upon receipt of RCSD contribution--$22,307 <br /> Third payment February 1,2015--$158,997 <br /> IX.Confidentiality <br /> All personal identifying student information will remain strictly confidential and will not be shared with any other <br /> entities without the express written authorization of representatives of all parties,except as otherwise required by law <br /> or necessary to effectuate the purposes of this Agreement. <br /> X. Procedures for Resolving Disputes <br /> In the event of a dispute,difference of interpretation or appeal of a decision regarding the terms and/or conditions of <br /> this Agreement,settlement shall first be sought through discussion with SHD's Healthy Schools Initiative Director and <br /> CITY's Parks, Recreation, and Community Services Director. If settlement cannot be reached,the issue shall then be <br /> presented to the Chief Executive Officer of SHD and the City Manager of Redwood City,for joint resolution by them. <br /> XI. Record Keeping and Reporting <br /> CITY shall maintain sufficient accounting records regarding the expenditure of funds and permit inspection of the <br /> records if requested.An interim report shall be presented to the SHD Healthy Schools Initiative Director in January of <br /> each grant cycle describing the implementation of the PE+Program,the number of coaches hired,the schools where <br /> seNices are being provided,and the number of students being seroed. Funding of the second portion of the annual <br /> grant is contingent upon receipt of a satisfactory interim repo�t.An annual narrative report on the program or projects <br /> carried out with these funds shall be delivered in writing along with a brief presentation to the SHD Board of Directors <br /> in June of each funding cycle(July 1 -June 30). The report should also include the number of students served and <br /> metrics of evaluation,which will be collected and reported in collaboration with PCC. SHD will maintain a separate <br /> operating budget and monitor expenses. Funding for the next grant cycle will be contingent upon receipt of a <br /> satisfactory annual report. SHD may also request additional financial expense reporting on a monthly or quarterly <br /> basis. <br /> Reporting Schedule: <br /> 1) First report due January 23, 2015 <br /> 2) Final report due June 19,2015 <br /> XII.
