Exhibit C Additional Provisions (Valid Unless Crossed Out and Initialed by CLIENT) Reports At no additional cost to CLIENT, STAFFING FIRM will generate and deliver to CLIENT the following reports, at the indicated intervals: Monthlv report of hours employee works Background Checks At STAFFING FIRM's expense, STAFFING FIRM will perform the following types of background / qualification checks for all employees which it selects for assignment to CLIENT and will not assign unqualified personnel to CLIENT: This report will investigate the previnus seven (7) years of the applicant's or employee's background. Additionally, the officia! records for the last three (3) counties in which the applicant or employee last lived will be investigated. Standard investigative consumer reports will entail 1) Social Security check, 2) Criminal History, and 3) National Wants&Warrants. STAFFING FIRM will be represented at CL.IENT's premises by an On-Site Coordinator. CLIENT will have the continuing right to reasonably ciisapprove of the person designated for this role by STAFFING FIRM and to require a suitabie replacement. STAFFING FIRM will also arrange for substitutes to cover the On-Site Coordinator's material absences during CLIENT's business or other operational hours. CLIENT will provide the On-Site Coordinator with the necessary office space, furnishings, and other resources as the parties may agree on from time to time. The On-Site Coordinator's duties will be as follows: Guarantee The Assigned Staff that STAFFING FIRM recruits and assigns to CLIENT will have the qualifications CLIENT requests. If CLIENT finds any Assigned Employee's qualifications or general work-related behavior lacking and lets STAFFING FIRM know within 1 business day of the assignment, STAFFING FIRM will not charge CLIENT for the first day of the assignment and will make reasonable efforts to replace the Assigned Employee immediately. If at any time, CLIENT is unhappy or dissatisfied with Assigned Staff, for any reason or no reason, CLIENT may notify STAFFING FIRM that the services of that particular Assigned Staff are no longer acceptable and not to provide such Assigned Staff to CLIENT any further, after the date of such notice, but instead to send a different Assigned Staff as soon as reasonably possible. Insurance Throughout the term of this Agreement, STAFFING FIRM shall obtain and maintain, at its own expense, the minimum insurance coverage set forth below. The City and its elected officials, officers, and employees shall be named as additional insured on all policies except Worker's Compensation: ATTY/AGR/2012.101/PRIMUS STAFFING SOLUTIONS INC.
