,•.. <br /> Exhibit C <br /> Additional Provisions (Valid Unless Crossed Out and Initialed by CLIENT) <br /> Reports <br /> At no additional cost to CLIENT, STAFFING FIRM will generate and deliver to CLIENT the following <br /> reports, at the indicated intervals: Monthlv report of hours employee works <br /> Background Checks <br /> At STAFFING FIRM's expense, STAFFING FIRM will perform the following types of background / <br /> qualification checks for all employees which it selects for assignment to CLIENT and will not assign <br /> unqualified personnel to CLIENT: <br /> This report will investigate the previnus seven (7) years of the applicant's or employee's <br /> background. Additionally, the officia! records for the last three (3) counties in which the <br /> applicant or employee last lived will be investigated. Standard investigative consumer reports <br /> will entail 1) Social Security check, 2) Criminal History, and 3) National Wants&Warrants. <br /> STAFFING FIRM will be represented at CL.IENT's premises by an On-Site Coordinator. CLIENT will <br /> have the continuing right to reasonably ciisapprove of the person designated for this role by <br /> STAFFING FIRM and to require a suitabie replacement. STAFFING FIRM will also arrange for <br /> substitutes to cover the On-Site Coordinator's material absences during CLIENT's business or other <br /> operational hours. CLIENT will provide the On-Site Coordinator with the necessary office space, <br /> furnishings, and other resources as the parties may agree on from time to time. The On-Site <br /> Coordinator's duties will be as follows: <br /> Guarantee <br /> The Assigned Staff that STAFFING FIRM recruits and assigns to CLIENT will have the qualifications <br /> CLIENT requests. If CLIENT finds any Assigned Employee's qualifications or general work-related <br /> behavior lacking and lets STAFFING FIRM know within 1 business day of the assignment, <br /> STAFFING FIRM will not charge CLIENT for the first day of the assignment and will make <br /> reasonable efforts to replace the Assigned Employee immediately. <br /> If at any time, CLIENT is unhappy or dissatisfied with Assigned Staff, for any reason or no reason, <br /> CLIENT may notify STAFFING FIRM that the services of that particular Assigned Staff are no longer <br /> acceptable and not to provide such Assigned Staff to CLIENT any further, after the date of such <br /> notice, but instead to send a different Assigned Staff as soon as reasonably possible. <br /> Insurance <br /> Throughout the term of this Agreement, STAFFING FIRM shall obtain and maintain, at its own <br /> expense, the minimum insurance coverage set forth below.
