6.1.E. - Page 159 QRfGINAL <br /> FuNDING A G R E E M � N T FOR HUMAN SERVICES FINANCIA�ASSISTANCE PROGRAM <br /> B�TWEEN THE CITY OF RE�WOQD CITY AND <br /> SERVICE LEAGlJE OF SAN MATEO COUN7Y <br /> 7'H15 AGFtEEMENT, made and entered into this day of , 2012, by and <br /> between the CI�I' OF R�[]WOOD CITY, a charter city and a municipal corporation of the State of <br /> CaEifornia ("City"} and Service League of San Mateo County, a California nonproft organization <br /> ("Contractar"}; <br /> WITN � SSETH: <br /> WHEREAS, Contractor will provide certain services through programs, including the program <br /> dsscribed in Exhibit"A" ("Contract Objectiv�s"), and <br /> WHEREAS, Contractor has requested financiai assistance in order tn conduct the program for <br /> residents of City during the fiscal year commencing July 9, 2012, and ending June 3D, 2013; and <br /> WHEREAS, City has re�iewed said request and de5ires to allocate to Contractor the sum of <br /> $5,000; <br /> NOW, 7H�REFOR�, the parties hereta do hereby agree as follows: <br /> 1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Fi�e Thousand <br /> Dollars for exclusi�e use by Cnntractor during the fiscal year commencing July 9, 2012, and ending June <br /> 3Q, 2093, to be used by Contractor solely for the purposes described in paragraph 2. <br /> 2. USE OF FUNDS. Contractor shall use the funds pro�ided pursuant to paragraph 1 solely <br /> #or the Program purposes described in Exhibit "A" (Contract Qbjeetives) attached hereto and by this <br /> reference incorporatecf herein. <br /> 3. CHANGES TO PROGRAM. No changes in the Program described in this agreement which <br /> is funded by the financial assis#ance pro�ided under paragraph 1 shall be made without the prior written <br /> consent of City. <br /> 4. PAYMEN7S. Contractor shal! keep detailed and accurate recards of all expenditures made <br /> and expenses incurred which are €unded under thi5 agreement.
