EXHIBIT "A" <br /> SCOPE OF SERVICES, TIME AND FEE SCHEDULES <br /> PROJECT DESCRIPTION: <br /> Geotechnical inspection and testing services at various Redwood City locations on an as- <br /> needed basis for 2011-2012 Sanitary Sewer Replacement Project. <br /> SCOPE OF SERVICES: <br /> Bay Area Geotechnical Group Engineers shall provide all required labor, material, and <br /> equipment to conduct geotechnical inspection and testing services for the 2011-2012 <br /> Sanitary Sewer Replacement Project. <br /> The Consultant shall: <br /> 1. Pertorm laboratory material acceptance standard test which includes a gradation <br /> test, compaction curve, R-Value test, sand equivalent test, and durability index test. <br /> 2. Perform field observations/field density compaction testing services (nuclear gauge) <br /> as requested, and on an intermittent basis for trench bedding and backfill in <br /> compliance with the project plans and specifications. <br /> 3. Document test data and observations through daily and end-of-job summary reports. <br /> Daily reports shall be submitted to the City on a daily basis. Daily reports should <br /> describe the activities of the contractor at the time of consultanYs field services, and <br /> the locations and results of consultant's nuclear field density tests. <br /> Deliverables: Summary Inspection Reports <br /> TIME SCHEDULE: <br /> The schedule for the work shall be on an as-needed basis and consultant shal I be available <br /> to perForm on-site with a minimum of two (2) hours notice by City. <br /> FEE SCHEDULE: <br /> Consultant shall provide services outlined above for a not to exceed fee of $9,095.00 <br /> as follows: <br /> Geotechnical Inspection and Testinq Services: <br /> Field Technician, 95 hours, @ $85.00/hr. $8,075.00 <br /> (Consultant shall be paid a minimum of two hours for a site visit. Consultant shall bill any <br /> site visit longer than two hours based on actual time spent on site.) <br /> Laboratorv Testina: <br /> 1 Gradation Test $ 110.00 each...$110.00 <br /> 2 Compaction Curves $ 200.00 each...$400.00 <br /> 1 R-Value Test $250.00 each...$250.00 <br /> 1 Sand Equivalent Test $ 120.00 each...$120.00 <br /> 1 Durability Index Test $ 140.00 each...$140.00 <br /> 1 020.00 <br /> Total Inspection and Testing Cost $9,095.00 <br /> Amounts payable within 30 days of City's receipt of acceptable deliverables. <br /> ATTY/AGR/2012.138/BAGG 9095 <br /> REV:09-05-12 VR <br /> Page 4 of 4 <br />
