Page 1 of 4 �� <br /> � <br /> AGREEMENT FOR SERVICE BETWEEN THE REDVVOOD CITY <br /> SCHOOL DISTRICT AND CITY OF REDWOOD CITY ° <br /> This agreement is made as of this day of June 27, 2012 by and between the Redwood City <br /> School District("RCSD")and City of Redwood City("CONTRACTOR"}to provide services. <br /> b <br /> NOW,THEREFORE,THE PARTIES AGREE AS FOLLOWS: <br /> 1. SERVICES <br /> The CONTRACTOR agrees to provide said services to the RCSD in accordance with the <br /> terms and conditions af this agreement. <br /> A. Scope of Services <br /> The CONTRACT4R agrees to provide extended day academic and enrichment <br /> services described in RCSD Extended Day Program & School Minimum ' <br /> Expectations in Attachment A and RCSD Extended Day Policies and Protocols in <br /> Attachment B at Fair Oaks, Garfield, Hawes, Henry Ford, Hoover, John Gill, and <br /> Selby Lane. The RGSD wiil provide the necessary supports to the CONTRACTOR <br /> as descrihed in Attachment A. The CONTRACTOR, if applicable will refer 21 S` <br /> Century Community Learning Center famities to existing literacy and related <br /> educational development services. <br /> � <br /> The CONTRACTOR agrees to utilize the computer system (CitySpan) provided by � <br /> the RCSD to enter daily attendance. Any technical difficulties associated with <br /> utilizing this system will be reported in a timely manner within 72 hours. � <br /> � <br /> 2. EQUII'MENT AND MA.TERIALS � <br /> CONTRACTOR shall provide all equipment, materials, and supplies necessary for the � <br /> performance on the agreement. � <br /> �. <br /> 3. USE OF SUBCONTRACT(?RS <br /> CONTRACTOR sha11 not assign this agreernent or any portion thereof to a third party � <br /> without the prior written consent of RCSD. � <br /> � <br /> � <br /> 4. LICENSES AND PERMITS � <br /> CUNTRACTOR shall obtain and keep in force all licenses, permits, and certificates <br /> necessary for the performance of this agreement. ; <br /> 5. INSURANCE <br /> CONTRACTOR understands the RCSD does not provide liability, malpractice, ar � <br /> workers compensation inswance for the CONTRACTOR. The CONTRACTOR is <br /> responsible for the maintenance of personai and professional insurance to provide <br /> protection for any claims arising out of service under this agreement. The <br /> CONTRACTOR assumes all liability for services provided under this contract.
