<br /> ~----- <br /> ORIGINAL <br /> AGREEMENT FOR WORKERS' COMPENSATION <br /> CLAIMS ADMINISTRATION SERVICES <br /> This Agreement is made effective this 1 sl day of February 2002, between Claims <br />Management, Inc. (hereinafter referred to as "CMI"), and the City of Redwood City <br />(hereinafter referred to as "City"). <br /> RECITALS <br /> WHEREAS, City desires to self-insure its liability to its employees pursuant to the <br />Workers' Compensation laws of the State of California, and <br /> WHEREAS, CMI is qualified and willing to administer the self-insured workers' <br />compensation program undertaken by the City, and <br /> WHEREAS, City desires to retain the services of CMI and CMI agrees to render <br />its services for City on the terms and conditions of this Agreement. <br /> NOW, THEREFORE, in consideration of the mutual promises, conditions and <br />covenants hereinafter set forth, CMI and City agree as follows: <br />I. TERM <br /> The term of this Agreement shall be from 12:01 a.m., February 1, 2002 to <br />midnight on June 30, 2002, unless sooner terminated as provided in Section IV, hereof. <br />II. CONSIDERATION <br /> A. The annual fee payable to CMI pursuant to this Agreement shall be <br />$108,000, payable in arrears in monthly installments of $9,000, which payment shall be <br />due on the first day of each month beginning March 1, 2002. <br /> B.
