<br /> - <br />made a part hereof. In the event City desires to amend said services, City may submit <br />said amendment to CMI for approval. <br />VI. CLAIMS ADMINISTRATION <br /> CMI shall provide the following workers' compensation claims management for <br />City: <br /> A. Review on behalf of City, all reports of injury as defined by Labor Code <br />Sections 3208 and 3208.1, as the same now exist or may be amended, and make <br />contact with City's injured employee(s) within 24 hours of City's notice to CMI of City's <br />employee(s) lost time injury. <br /> B. CMI shall, for each reported injury, determine those benefits, if any, that <br />should be paid or rendered under the California Workers' Compensation laws. City is <br />responsible for completion of Employer's Report of Injury within five (5) days from the <br />date of knowledge of alleged injury. City will be responsible for any penalties or <br />assessments arising from any claim City does not report to CMI within five (5) days from <br />the date of knowledge of alleged injury. <br /> C. CMI shall establish and maintain an orderly claims file on each reported <br />claim. Each file shall be available to City for inspection, with or without prior notice to <br />CMI. <br /> D. CMI shall maintain an estimate of the total cost of all reasonably <br />foreseeable benefits and related expenses on each case. Each cost shall be recorded <br />separately in the following categories: <br /> 1) Medical <br /> 2) Indemnity <br /> 3) Allocated <br /> 4) Rehabilitation <br />F:A TTY/AGR/AGR007 3 <br />010802/STY.cls <br />
