<br />J. No later than the tenth (10Ih) day of each calendar month, CMI shall <br />provide to City a monthly check register of all financial transaction performed during the <br />preceding calendar month. <br />K. No later than the tenth (10Ih) day of each calendar month, CMI shall <br />provide to City a computerized loss analysis and summary, including cumulative loss <br />information, covering activity of claims reported to CMI during the preceding month. <br />L. CMI's employees shall perform field investigations on individual claims <br />upon the mutual agreement of both parties. All costs and fees for these services will be <br />paid at the standard billing rate, which rate is shown on Exhibit "B, " which is attached <br />hereto and made a part hereof, plus $.50/per mile. Said costs and fees shall be paid <br />under allocated expenses after approval by City. <br />M. Horizon Managed Care, LLC, an affiliated entity, is CMl's on-site copy <br />services. Allowable photocopy charges are billed to the file under allocated loss <br />expenses. <br />N. CMI will fully cooperate with all audit requests by City or other associated <br />agencies. <br />O. Affiliated Entities: Horizon Managed Care, LLC ('HMC") and Single <br />Source Medical, LLC ("Single Source Medical") are owned by shareholders and <br />executives of CMI. <br />HMC provides a medical cost management program. HMC shall be paid by City <br />for bill review and other services rendered by staff for the managed care program. <br />Costs for outside services for managed care shall also be paid by City. <br />F:ATTY/AGR/AGR007 5 <br />010802/STY.cls <br />
