.• EXHIBIT "A" SCOPE OF SERVICES Redwood City/North Fair Oaks AOD Abuse Prevention Partnership 2012-2013 Budget Deliverables Worksheet Name of Contractor: PCRC Funding Amount:$37,000 Deliverables • Facilitate Monthly Prevention Partnership Meetings a. Agenda development, logistical coordination, notes and next steps • Facilitate 4-6 Community Meetings/gatherings/events a. Provide coordination, planning,facilitation,follow-up, and logistics for community engagement events at least once a Quarter • Facilitate connection between PCRC resources,specificaliy but not limited to Family Engagement staff and Pacific Islander Community. • Ensure ongoing communication with contracted and nan-contracted partners with monthly in person or phone conversations • Provide 1-2 meetings with each individual contracted partner identifying milestones, solidifying contract agreements and conducting year end reviews • Ensure appropriate County reporting and data collection requirements are met including but not limited to attending monthly Provider meetings, entering and maintaining Prevention Partnership monthly CaIOMS records, Quarterly Reports and Annual Reports. • Work with staff at John W. Gardner Center to identify indictors, develop and implement evaluation plan for tracking success and impact. • Document efforts of Prevention Partnership during grant year to track indicators. • Work collaboratively with RWC 2020 Executive Director in update RWC2020 website and messaging and marketing materials by December 2012. Method of Payment PCRC will provide detailed invoices,with evidence of progress in achieving defined deliverables when requesting payment. The contract will be paid in two installments: • First instaliment to be invoiced by December 31, 2012 • Final installment to be invoiced upon completion of the project.
