EXHIBIT "A" SCOPE OF SERVICES Redwood City/North Fair Oaks AOD Abuse Prevention Partnership 2012-2013 Budget Deliverables Worksheet Name of Contractor:Center for Youth Funding Amount:$21,000 Deliverables • Conduct AOD prevention education sessions for approximately 600 students in 10tn 11`h and 12th grade Biology classes for 2 weeks per class at Sequoia High School. • Develop session on media literacy and the media's influence on young peoples' decisions around AOD use, incorporate into CFY's AOD prevention education sessions,and conduct in classroom sessions(described above). • Work in collaboration with after school groups of the Sequoia Teen Resource Center (e.g.Youth Advisory Board)to plan and conduct weeklong activities and presentations on alcohol and other drugs reaching approximately 300 students during"AOD Week" in April 2013. • Develop and conduct media literacy related to prevention of AOD use/abuse for approximately 300 students at the Health Fair at Sequoia High School in March 2013. • Work in collaboration with the Photovoice Workgroup of the AOD Prevention Partnership on projects for 2012-13. • Conduct evaluations of the classroom sessions and peer educator trainings by developing, administering and coding pre-surveys and post-surveys to approximately 400 students in the 10th, 11th and 12th grade at Sequoia High School. Method of Payment CFY will provide detailed invoices,with evidence of progress in achieving defined deliverables when requesting payment. The contract will be paid in two installments: • First installment to be invoiced by December 31, 2012 • Final installment to be invoiced upon completion of the project.
