EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Redwood City/North Fair Oaks AOD Abuse Prevention Partnership <br /> 2012-2013 Budget Deliverables Worksheet <br /> Name of Contractor:StarVista <br /> Funding Amount:$20,000 <br /> Deliverables <br /> • StarVista Program Manager will oversee the Youth Access Workgroup to include furthering the TRACE <br /> project;to bring awareness to members of the community via media,summer movie PSA,stores,back <br /> to school night,middle school youth to youth.The outcome of this project will be presented to the <br /> Prevention Partnership and RWC/NFO Law Enforcement. <br /> • StarVista will lead community education Campaign reaching Sequoia High School, Redwood City Stores, <br /> 400 parents and community members <br /> • StarVista staff and Youth Advisory Board(YAB)will administer a Needs Assessment survey to more than <br /> 800 students at Sequoia High School and present the results to the Prevention Partnership and School <br /> Administration by the end of the 2012-2013 school year(in Spring 2013) <br /> • Share process for conducting Needs Assessment with Prevention Partnership Coordinator <br /> • YAB youth will be present for non-steering committee Prevention Partnership meetings.Two YAB youth <br /> will serve on the Prevention Partnership Steering Committee,and attend required meetings every <br /> alternate month. <br /> • StarVista staff will plan and conduct AOD activities on three days on the campus of Sequoia High School <br /> to include Day of Silence/Day of Voices,connection with TRACE Campaign and other activities around <br /> AOD.Each day will have a different activity on campus.We estimate that we touch approximately 1,200 <br /> students on campus during this week(in Spring 2013). <br /> • StarVista will staff weekly YAB meetings at Sequoia High School,using Youth Development Theory as <br /> the foundation of the work.The YAB will make at least two presentations to the Prevention Partnership <br /> before the end of the 2012-2013 school year on Needs Assessment and TRACE Campaign <br /> accomplishments. <br /> • Participation by senior management in the RWC/NFO AOD Prevention Partnership to include monthly <br /> meetings,and any other activities being conducted(year-long) <br /> • CEO of StarVista will continue to work with the Partnership on reducing access to alcohol in our <br /> communities,working with law enforcement,and possibly city officials to strengthen existing penalties <br /> for selling alcohol to minors and allowing minors to acquire it through other means. CEO will continue <br /> to work with corner-store makeover committee to evaluate the success of the program and if <br /> necessary, revise the program so it can be successful and sustainable. <br /> Method of Payment <br /> StarVista will provide detailed invoices,with evidence of progress in achieving defined deliverables when <br /> requesting payment. <br /> The contract will be paid in two installments: <br /> • First installment to be invoiced by December 31,2012 <br /> • Final installment to be invoiced upon completion of the project. <br /> ATTY/AGR/2012.196/AOD PROGRAM MANAGEMENT STARVISTA FY 12-13 <br /> REV:12-14-12 PT <br /> Page 10 of 10 <br />
