Exhibit A <br /> (Program and Project Description) <br /> PROGRAM/PROJECT DESCRIPTION & REQUIREMENTS <br /> 1. Proiect Description and Elipible Costs: Funds hereunder are provided pursuant to <br /> 24 CFR 92, regulations covering the use of HOME funds for eligible CHDO activities. <br /> HOME CHDO funds of $92,197.00 provided under this Agreement shall be used to <br /> assist in eligible costs to complete the rehabilitation of rental housing and conversion of <br /> one unit to meet Section 504 accessibility requirements on property located at 1157- <br /> 1161 Willow Road, Menlo Park, California ("ProjecY' or "Property"). HHDC shall use the <br /> funds provided hereunder for the advancement of programs that provide affordable <br /> housing to very low-income residents of the City. <br /> The HOME-assisted rental housing Property rehabilitated with the HOME funds <br /> provided under this Agreement will meet all applicable state and local codes, local <br /> rehabilitation standards and ordinances, and accessibility requirements of the Fair <br /> Housing Act and Section 504 of the Rehabilitation Act of 1973. Contractor must <br /> maintain Property in accordance with property standards throughout the affordability <br /> period., in addition to all other applicable laws, ordinances, rules and regulations of <br /> federal, state, county or municipal governments or agencies now in force or that may be <br /> enacted hereafter that govern the construction. <br /> The table below summarizes the unit breakdown of the completed Project, including the <br /> distribution and number of HOME-assisted units: <br /> Three (3) Proposed Rents for HOME- <br /> # Bedrooms/Unit Total # Units HOME-Assisted Assisted Units (Utilities paid by <br /> Units tenant) <br /> Low/Hi h HOME Rents <br /> 1 10 2 $988/$1,187 <br /> 2 2 1 $1,187 /$1,425 <br /> Total 12 3 <br /> The table below is a summary of the scope of property rehabilitation and the estimated <br /> Project budget.
