�! � I <br /> EXHIBIT A <br /> PROJECT DESCRIPTION <br /> Inspection services for the Block 2 Culvert Relocation Project located on Middlefield Road <br /> between Jefferson and Winslow adjacent to the Middlefield parking lot. <br /> SCOPE OF SERVICES <br /> Consultant shall provide the following services: <br /> • Understand the project and City's primary goals and objectives. <br /> • Coordinate project related information with City's Construction Manager, and city staff with <br /> information, insight and potential solutions as conflicts and problems arises. <br /> • Familiarize him/herself with the City's standards and plans, as well as Caltrans standards and <br /> plans. <br /> • Take digital photos of each stage of construction throughout the duration of the project. <br /> • Prepare and distribute written daily field inspection reports via e-mail and/or hard copy. <br /> Daily field reports shall be detailed and contain all pertinent information, including labor <br /> equipment (including idles), items of work and its description, any contract change order <br /> work and accurate hours associated with it and daily quantity measurements. <br /> • Report instances of apparent non-compliance to the City's Construction Manager for <br /> resolution. <br /> • Review contractor's monthly progress payment application with City's Construction <br /> Manager. <br /> • Prepare punch lists and conduct final inspections. <br /> PAYMENT TERMS <br /> Consultant shall charge a standard hourly rate of $90/per hour for inspection services, which <br /> services shall be complete on or before June 30, 2013, and total compensation for those services <br /> shall not exceed Fifty Thousand and 00/100 Dollars($50,000.00). <br /> ATTY/AGR/2013.036/General Service Agreement/CSG Consultants,Inc. <br /> REV:03-08-13 VR Page 5 of 5 <br />
