EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> Funding for this agreement has been granted from the San Mateo County Health System to <br /> support the coordination and communication for the implementation phase of the NFO <br /> Community Plan. The Consultant (North Fair Oaks Community Plan Communications <br /> Coordinator Lilia Ledezma and Community Consultants Group) will play a lead role in <br /> coordinating activities across a diverse set of stakeholders that includes public agencies, <br /> business owners, residents, school leadership, community based leaders, youth and regional <br /> agencies. <br /> Specific tasks to be completed by Consultant during the Term shall include: <br /> • Develop and maintain a database of all North Fair Oaks (NFO) stakeholders; <br /> • Work with Communications Work Group to develop and carry out a NFO Communication <br /> Plan including: <br /> o regular newsletter; holding update sessions; e-blasts/e-newsletter; website or <br /> other"one stop shop"; and <br /> o Facilitate the Communications Workgroup; and <br /> o Publicize successes through newspapers and radio; and <br /> o Track implementation activities and serve as the "go to" person on all related <br /> activities— person doesn't need to be involved in implementation at all levels, but <br /> needs to know what is going on and how to connect people; and <br /> o Maintain a master calendar of implementation workgroups/related <br /> implementation activities and publicizing this calendar; and <br /> o Work with the other workgroups to develop timelines and brainstorm <br /> implementation strategies and "connecting the dots" across workgroups; and <br /> o Recruit community member participation for workgroups and connect interested <br /> community members to implementation activities; and <br /> o Staff the Quarterly Implementation Coordination Meetings including agenda <br /> preparation, promotion of the meeting and maintaining documentation in <br /> coordination with Oversight Group; and <br /> o Identify issues and areas where progress is not taking place and bring these <br /> concerns to the Oversight Group; and <br /> o Identify who is responsible for areas of implementation that are community <br /> priority and identify for moving these forward where problems arise bringing <br /> these forward. <br /> Consultant will invoice RWC 2020 monthly, providing documentation of actions taken during that <br /> proceeding month. <br /> The total maximum fee payable for the Services to be perFormed during the initial Term of this <br /> Agreement shall be $23,000.00. <br /> ATTY/AGR/2013.033/RC2020 LILIA LEDEZMA NFO COMMUNITY PLAN <br /> REV:03-01-13 MLG <br /> Page 10 of 10 <br />
