• <br /> BAAQMD FYE 2013 TFCA Funding Agreement <br /> ATTACHMENT C <br /> MONITORING OF PROJECT PERFORMANCE <br /> 1. Semi-annual Reports (Section II.8): The Project Sponsor shall submit Semi-annual Reports to the <br /> Air District summarizing Project progress. Semi-annual Reports shall be prepared on the Air <br /> District's Semi-annual Report form. <br /> Due Dates: By April 15 and October 15, 2013. <br /> 2. Final Report(Section II.8): <br /> Due Date: March 31, 2014 <br /> The Final Report must be submitted on the Air District's Final Report form, and shall include: <br /> A. The following data, taken from responses of the ridership survey, for each route listed in <br /> Attachment B of the Funding Agreement (if more than one route, use a table to display <br /> information). The survey must include the questions provided in the Air District Annual <br /> Shuttle/Feeder Bus Ridership Survey form: <br /> 1. Average distance from home to the start-of-commute transit station; <br /> 2. Mode of transportation from home to the transit station; <br /> 3. Average distance from home to the final destination; and <br /> 4. Percent (%) of people that would drive if the shuttle/feeder bus were not available. <br /> B. The following information for each route listed in Attachment B of the Funding Agreement: <br /> 1. Route map and schedule; <br /> 2. Total annual boardings (for previous 12 months of operation); and <br /> 3. Total number of days of service; <br /> C. The following vehicle information for each vehicle included in each route listed in Attachment <br /> B of the Funding Agreement: <br /> • Executive Order • Retrofit device used for any diesel vehicles <br /> • Revenue miles of service • Passenger capacity of the vehicle(s) used to provide the shuttle service <br /> • Gross vehicle weight <br /> D. A discussion of any pertinent issues or problems that arose during the Project, plan to alter <br /> and/or continue the shuttle service, and the proposed funding source(s) for continued service. <br /> E. Documentation that the Project Sponsor has acknowledged the Air District as a Project funding <br /> source during the Project Useful Life, including photographs of vehicles operated as part of the <br /> Project with Air District logos attached; documentation of use of the logo on the Project <br /> Sponsor's website, promotional materials, and on Project transit schedules, brochures, <br /> handbooks, and maps that promote ar inform the public about the Project services; and copies <br /> of press releases and newsletter articles related to the Project(Section II.l 1). <br /> F. The following data(in Excel) as attachments to the report: <br /> 1. Annual Shuttle/Feeder Bus Ridership Survey results; and <br /> 2.
