Healthy Schools Ini#iative >'�r.e,ra He;�i.'hr.are D;�tr�,:* 2013 - 2014 Healthy Schools Grants Program Grant Agreement Sequoia Healthcare District is pleased to award the City of Redwood City,acting as fiscal agent for Redwood City 2020,a one-time grant of$10,000 to support the Safe Routes to School Initiative Climate Corps Volunteer during the 2013/2014 school year. The grant is for the explicit purposes outlined in your grant proposal and is subject to the conditions listed below. To acknowledge this agreement and accept the grant,please have an officer of your organization read and sign this agreement and return it to us at Sequoia Healthcare District,525 Veterans Blvd.,Redwood City,CA 94063 by June 14,2013. Grant Number: 2013-14-003 Grant Amount: $10,000 Grant Period: July 1,2013-June 30,2014 Grant Purpose: Climate Corps Volunteer Payment Schedacle: First payment July 1,2013--$5,000 Second payment February 1,2014--$5,000 Reporting Schedule: 1) First report due January 20,2014 2) Final report due June 20,2014 Special Conditions: Sequoia Healthcare District Representative may request a site visit during the grant cycle. I. Expenditure of Funds The grantee shall use this grant solely for the purposes stated above,and the program may be modified only with the written approval of the Sequoia Healthcare District's Board of Directors. The grantee shall return to the District any unexpended funds: 1) at the end of the grant period,or 2) if the Sequoia Healthcare District determines that the grantee has not performed in accordance with this agreement and the approved program and budget,or 3) if the grantee loses exempt"Public Charity" status under Section 501 (c) (3) of the Internal Revenue Code. The grantee shall maintain adequate supporting records consistent with generally accepted accounting practices to show the use of these funds. The grantee shall make these books and records available to the Sequoia Healthcare District in keeping with the grant reporting schedule described below. The grantee shall not use any portion of the funds granted herein to influence the outcome of any specific public election. This grant is not earmarked for lobbying purposes and no portion of it may be used for lobbying purposes.
