Proposed Senior Assisted Living Facility <br /> PSI Project No. 575-308-2 <br /> August 3, 2011 <br /> Page 2 <br /> 2.0 PROJECT INFORMATION <br /> 2.1 Proiect Authorization <br /> Professional Service Industries, Inc. (PSI) is pleased to submit our Geotechnical Engineering <br /> Services Report for the proposed Senior Assisted Living Facility at the subject site. Our <br /> evaluation was completed in general accordance with the scope of work as outlined in our <br /> Proposal Number 575-44659, dated June 10, 2011. Written authorization was provided by Mr. <br /> William Shields of Fountain Square Development West, LLC., on June 24, 2011. <br /> 2.2 Site Location and Description <br /> The subject property is located adjacent to the southwest corner of Oakwood Drive and EI <br /> Camino Real in Redwood City, San Mateo County, California. The Subject Property is defined <br /> by the County of San Mateo Assessors Office as Assessors Parcel Numbers (APNs) 059-172- <br /> 070, 059-172-370, and 059-172-380. The site is a roughly rectangular-shaped property <br /> measuring approximately 45,000 square feet in plan area. At the time of our fieldwork, the site <br /> was developed with two buildings, Jackson Fine Jewelers and a former restaurant, both located <br /> in the eastern portion of the site. The two buildings share a highly distressed asphalt parking lot <br /> and limited landscaping. The southwestern portion of the subject property is undeveloped other <br /> than two small sheds and is separated from the two buildings by a fence. The property slopes <br /> gently downward to the north, with an elevation (estimated from the Palo Alto, California USGS <br /> topographic map) of approximately 38 feet above mean sea level. <br /> 2.3 Proiect Understandinq <br /> PSI understands that it is currently planned to develop the site with a two-story wood frame, <br /> slab on grade building for use as a senior care facility at the subject site. The structure is <br /> planned to be approximately 16,000 square feet in plan area. Additional improvements will <br /> include associated utilities and asphalt-paved parking. An expected maximum column load of <br /> 40 kips and typical continuous footing load of 1,800 plf (dead loads) were provided. A scaled <br /> site plan with the location of the proposed building and improvements indicated was provided <br /> for our use by William Shields via Steinberg Architects. <br /> The finished floor elevation of the building was not furnished to us, but for the purposes of this <br /> report, we have assumed that cuts and fills will not exceed 2 feet. Any changes to the project <br /> from that stated above will need to be addressed at the time the information becomes available. <br /> 2.4 Purpose and Scope of Services <br /> The purpose of our geotechnical evaluation was to assess the subsurface soil conditions at the <br /> site in order to provide appropriate recommendations for site preparation and foundation design. <br /> Our evaluation was in general accordance with the scope of work outlined in our Proposal <br /> Number 575-44659, dated June 10, 2011. <br /> Our scope of services included six (6) Standard Penetration Test (SPT) borings, drilled to <br /> depths of between approximately 6-'/2 and 51-'/2 feet below the existing ground surface, two (2) <br /> hand-augered borings, and the preparation of this geotechnical report.
