a) Work with about fifty (50) referred youth during the 2012-13 school years, under the supervision of the Sequoia Teen Resource Center Director and in collaboration with the school administration. b) Complete a baseline profile for each student entering the program. c) Develop a written action plan to address the specific issues that interfere with the student's school success. Every pian will contain measurable goals such as consistent attendance at classes, completion of assignments, improvement in grades/behavior, etc. d) Meet at least weekly with each assigned student to revisit the goals of the action plan and ensure focus on goal attainment. e) Provide school staff members who refer the students with weekly updates regarding the studenYs participation, efforts, issues, progress etc. fl Help students establish contact with onsite and offsite student support services that promote school connectedness and success. g) Follow up with referrals for support services to ensure that assigned students have made connections and are continuing satisfactorily. h) Facilitate delivery of services through scheduling, helping with permission slips, and addressing other apparent barriers as appropriate. i) Maintain comptete, confidential and ongoing communication with appropriate school staff (teachers, counselors, administrators)as well as assigned students. j) Maintain complete records of steps taken to assist assigned students. k) Meet regularly with Director of Teen Resource Center and other AmeriCorps team members to continue to refine the design and implementation of the program. I) Complete other related duties as assigned by the school administration or Director of Teen Resource Center. 3. Fundinq. Redwood City 2020 will pay to Notre Dame a total fee for the volunteer's Services in the amount of$8,500. The maximum expenditure by Redwood City 2020 under this MOU is $8,500. No other compensation for the Services will be allowed, except for items covered by subsequent amendments to this Agreement. Redwood City 2020 reserves the right to withhold a ten percent (10%) retention until Redwood City 2020 has accepted the Services specified above. There shall be no additional charge for expenses unless agreed to by the Parties. Payment shall occur only after receipt by City of invoices sufficiently detailed to include hourly rates, hours worked, and tasks performed, unless otherwise agreed to by Redwood City 2020. Notre Dame will provide a detailed invoice, with evidence of progress in achieving defined deliverables when requesting payment. 4. Status of Volunteer. Notre Dame's volunteer will perform the Services as an independent contractor or employee of Notre Dame and not as an employee of City or Redwood City 2020. 5. Indemnification. Notre Dame will defend, indemnify and hold harmless City and its officers, agents, employees and volunteers and Redwood City 2020 and its officers, agents, employees, and volunteers, from and against all claims, damages, losses and expenses including attorney fees arising out of the performance of the Services, caused in whole or in part by the willful misconduct or any negligent act or omission of the volunteer or Notre Dame, any subcontractor, anyone directly or ATTY/AGR/2012.209/NORTE DAME MISSION VOLUNTEERS MOU—RC 2020 REV:12-21-12 MLG Page 2 of 4
