f <br /> . , Q�'��7 n�.�� <br /> AMENDMENT TO AGREEMENT <br /> BETWEEN THE COUNTY OF SAN MATEO <br /> AND <br /> CITY OF REDWOOD CITY–FAIR OAKS COMMUNITY SERVICE CENTER <br /> THIS SECOND AMENDMENT TO THE AGREEMENT, entered into this 23rd day of <br /> June, 2015, by and between the COLJNTY OF SAN MATEO, hereinafter called "County," and <br /> CITY OF REDWOOD CITY- FAIR OAKS COMMiJNITY SERVICE CENTER, hereinafter <br /> called "Contractor"; <br /> WITNESSETH: <br /> WHEREAS, pursuant to Government Code, Section 31000, County may contract with <br /> independent contractors for the furnishing of such services to or for County or any Department <br /> thereof; <br /> WHEREAS, the parties entered into an Agreement for Core Services on June 4, 2013; <br /> and <br /> WHEREAS,the parties amended that Agreement on April 11, 2014, to add the Cost of <br /> Living Adjustment(COLA); and <br /> WHEREAS, the parties now wish to further amend the Agreement to provide Core <br /> funding in the amount of $9,821 and Sustainability funding in the amount of $33,437 Fiscal <br /> Year 2014-2015. <br /> NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO <br /> AS FOLLOWS: <br /> 1. Section 1 –E�ibits and Attachments - of the Agreement is amended to read as follows: <br /> Exhibit A–Program/Project Description Revised 1/2015 <br /> Ezhibit B1- Method and Rate of Payment <br /> Exhibit C–Contractor's Declaration Form <br /> Exhibit D–Program Monitoring <br /> Exhibit E–Performance Measures <br /> Exhibit F–Clarity <br /> Attachment I—§504 Compliance <br /> 2. Section 2–Services to be performed by Contractor- of the Agreement is amended to <br /> read as follows: <br /> -1- <br />
