C�r�lg��aj <br /> AMENDMENT TO AGREEMENT <br /> BETWEEN THE COUNTY OF SAN MATEO <br /> AND <br /> CITY OF REDWOOD CITY—FAIR OAKS COMMUNITY SERVICE CENTER <br /> THIS THIRD AMENDMENT TO THE AGREEMENT, entered into this�day of <br /> , 0l6/(0 , by and between the COLTNTY OF SAN MATEO, hereinafter called <br /> "County," and CITY OF REDWOOD CITY - FAIR OAKS COMMIINITY SERVICE <br /> CENTER, hereinafter called "Contractor"; <br /> WITNESSETH: <br /> WHEREAS, pursuant to Government Code, Section 31000, County may contract with <br /> independent contractors for the furnishing of such services to or for County or any Department <br /> thereof; <br /> WHEREAS, the parties entered into an Agreement for Core Services on June 4, 2013; <br /> and <br /> WHEREAS, the parties amended that Agreement on April 11, 2014, to add the Cost of <br /> Living Adjustment (COLA); and <br /> WHEREAS, the parties amended that Agreement on June 23, 2015, to provide Core <br /> funding in the amount of$9,821 and Sustainability funding in the amount of $33,437 Fiscal <br /> Year 2014-2015. <br /> WHEREAS, the parties now wish to further amend the Agreement to add the FY 2015- <br /> 16 funds for Net County Cost (NNC) in the amount of$9,065, COLA in the amount of$2,205 <br /> and Sustainability Funds in the amount of$16,718, for a total amendment amount of$27,988 <br /> and a total agreement amount of$290,706. <br /> NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO <br /> AS FOLLOWS: <br /> 1. Section 1 —Exhibits and Attachments - of the Agreement is amended to read as follows: <br /> Exhibit A—Program/Project Description Revised 11/2015 <br /> Exhibit B1- Method and Rate of Payment Revised 11/2015 <br /> Exhibit C—Contractor's Declaration Form <br /> Exhibit D—Program Monitoring <br /> Exhibit E—Performance Measures <br /> Exhibit F—Clarity <br /> -1- <br />
