Funds <br /> shall be issued upon confirming implementation and positive review of performance <br /> data as approved by the County Manager. <br /> FY 2015-16 <br /> 1. At the discretion of the Director of the Human Services Agency, Contractor may be <br /> eligible to receive a one-time amount up to$16,718 during the term of the Agreement <br /> in sustainability funds following the evaluation of Performance Measurement data for <br /> the 1 S�quarter of FY 2015-16. Funds shall be issued upon confirming implementation <br /> and positive review of performance data as approved by the Director of the Human <br /> Services Agency. <br /> Total Agreement Obligation <br /> 2.
