EXHIBIT"A" SCOPE OF SERVICES Redwood City/North Fair Oaks AOD Abuse Prevention Partnership 2012-2013 Budget Deliverables Worksheet Name of Contractor:EI Centro de Libertad-Freedom Center Funding Amount:$4,000 Deliverables • Attend monthly steering committee meetings/provide information for dissemination to other participants around services EI Centro provides/particularly parent education groups • Participate in Youth Access Work group by organization leadership-attend scheduled meetings- provide meaningful input,identify opportunities for connecting parent groups to Youth Access efforts • Provide monthly 1%:hours parent education groups to approximately 6-10 parents/English and Spanish/incorporate principles of environmental prevention(eg: discussions of youth access to drugs and alcohol)-6 total • EI Centro will provide topics for the parent ed.Groups and the referral process to Redwood City Prevention Partnership • Coordinate with other members of the steering committee and other committees associated with RC2020/work collaboratively on relevant issues and share information from the Coastside Prevention Partnership • YAB will work with Mary Hofstead on the PhotoVoice Project • The PhotoVoice findings will be used in the Prevention Partnership Campaign • EI Centro will provide a meeting space for the RC/North Fair Oaks Prevention Partnership meetings • EI Centro de Libertad's Youth Advisory Board will share their PhotoVoice findings to community groups • RC2020 will provide a forum for EI Centro YAB to present PhotoVoice findings Method of Payment EI Centro will provide detailed invoices, with evidence of progress in achieving defined deliverables when requesting payment. The contract will be paid in one installment: • Payment to be invoiced upon completion of the project or by June 30,2013.
