EXHIBIT "A" <br /> SCOPE OF SERVICES AND FEE <br /> PROJECT DESCRIPTION: <br /> Inspection services for the Wharf Project. <br /> SCOPE OF SERVICES: <br /> Contractor shall provide the following services: <br /> � Understand the project and be familiar with the contract documents, City's standards <br /> and Caltrans standards. <br /> • Effectively communicate with the contractor, public and other stakeholders <br /> • Coordinate all project related information with City staff by providing timely information, <br /> insights and solutions as confli�ts and problems arise. <br /> • Take digital photos of each stage of construction throughout the duration of a project. <br /> • Prepare and distribute written daily inspection reports. Daily field reports shall have all <br /> pertinent irrformation, including labor, equipment(including idles), item of work, any <br /> change order worlc and accurate hours associated with it and daily quantity <br /> measurements. <br /> • Oversee special testing and inspection work as required. <br /> • Report instances of apparent non-compliance to the City's Project Manager for <br /> resolution. <br /> • Verify prevailing wages, payroll information and handle other labor compliance items <br /> when requested by the City. <br /> • Measure and agree upon quantities on a daily basis, and verify progress payments. <br /> • Deliver RFIs and submittals to the City's Project Manager. <br /> • Check as-built record drawings for accuracy. <br /> • Prepare punch lists and conduct final inspection. <br /> PAYMENT TERMS: <br /> Contractor shall charge a standard hourly rate of$110/per hour for inspection services on an as- <br /> needed basis, not to exceed Twelve Thousand Eight Hundred Fifty and No/100 Dollars <br /> ($12,850.00). Contractor's bill shall include the following information for the project: a brief <br /> description of services performed, the date the services were performed, the number of hours <br /> spent and by whom.
